PACE Resource Guide
When a California family begins exploring PACE, one of the first questions is: which organization provides the care? PACE is delivered by specialized PACE organizations that each serve defined geographic service areas — not by a single statewide plan.
California has multiple PACE organizations. The California Department of Health Care Services (DHCS) and the California PACE Association (CalPACE) maintain current directories of programs and service areas. This website is operated by an independent licensed agency that works with three of them: WelbeHealth, SCAN / myPlace Health, and InnovAge.

WelbeHealth is the largest PACE provider in California, operating 13 centers across the state. Founded with a mission to serve low-income seniors with complex medical needs, WelbeHealth now serves communities in Los Angeles County (including Pasadena, Carson, Long Beach, North Hollywood, and Rosemead), San Bernardino County, Riverside County, Sacramento County, Fresno County, San Joaquin County, Stanislaus County, Santa Clara County, and the Coachella Valley. WelbeHealth's model centers on adult day health centers where participants receive medical care, therapy, social activities, and meals, supplemented by in-home care and transportation.
SCAN Health Plan is one of California's oldest and most established Medicare health plans, and its PACE program operates under the myPlace Health brand. myPlace Health currently serves Los Angeles County, with centers in the Westlake / Central Los Angeles area and in South Los Angeles / Compton. SCAN brings decades of experience managing Medicare benefits for California seniors, and myPlace Health benefits from that infrastructure — including robust care management systems and deep experience with the dual-eligible population.
InnovAge is a national PACE organization with a significant California presence, operating centers in Los Angeles (Crenshaw), San Bernardino / Inland Empire, and Sacramento. InnovAge was among the first organizations to bring PACE to California and has a long history serving the state's most vulnerable seniors. In the Inland Empire and Sacramento, InnovAge and WelbeHealth both operate centers, giving families in those areas the option to compare programs.
