What Is PACE?

All the Care Your Parent Needs.
None of the Nursing Home.

The Program of All-Inclusive Care for the Elderly (PACE) is a federally and state-funded program that coordinates all medical and social services for adults 55 and older who need nursing-facility-level care but choose to remain at home.

A dedicated PACE care team handles everything — from doctor visits and specialist referrals to prescriptions, transportation, and in-home support — so your family doesn't have to navigate it alone.

Participants who qualify for both Medicare and Medi-Cal often pay $0 out of pocket for PACE services — including all covered care, prescriptions, and transportation. Final cost depends on individual eligibility.

Leslie is a licensed California insurance agent specializing in Medicare and PACE programs. As an independent advisor, Leslie works with three PACE partners — WelbeHealth, SCAN / myPlace Health, and InnovAge — rather than being limited to a single organization.

That means you receive guidance based on your loved one's needs and ZIP code. If your ZIP is not served by one of Leslie's partners, she will say so. Families in partner service areas can request a callback first to review options together. Leslie guides families through the process — from checking ZIP code availability to coordinating with a partner organization on enrollment — at no cost to you.
