We collect the following categories of personal information from this website:

• Inferences: service-area eligibility derived from your ZIP code.
• Internet activity: standard server log data (IP address, timestamp) collected when you submit the form, used solely to maintain a record of your consent.

We collect this information for the sole purpose of responding to your request for information about the Program of All-Inclusive Care for the Elderly (PACE) and Medicare options. We retain this information for as long as you remain an active prospect or client, and for at least seven (7) years thereafter, as required by California Department of Insurance record-retention rules.

We do not sell or share your personal information, and we do not use it for cross-context behavioral advertising. We do not rent, trade, or transfer your information to other agents, agencies, or marketing companies.

The phone number and email address you provide are used by Leslie Kaz, a licensed insurance agent (CA License # 0B32530) with Syndicated Insurance Agency, LLC, to:

• Send you a one-time SMS confirmation asking you to confirm your request.
• If you confirm by replying YES, contact you about PACE eligibility, Medicare Advantage, Medicare Part D, Medicare Supplement, and related options.
• If you do not confirm within 24 hours, mark your record as "do not contact" and take no further action.

At the moment you submit the form, we record a time-stamped consent record (the date and time, the IP address from which the form was submitted, and the exact text of the consent statement you saw). This record is kept solely to demonstrate compliance with the federal Telephone Consumer Protection Act and CMS marketing rules.

The only third parties to whom we disclose your personal information are:

• The PACE organization you ask us to refer you to, in the form of a warm introduction. Disclosure happens only after you have confirmed your request and only with your spoken or written instruction during your follow-up call.
• Service providers acting on our behalf under written contract, such as our SMS messaging provider, email provider, and CRM provider. These vendors are contractually prohibited from using your information for any purpose other than performing services for us.

We do not share your information with marketing partners, lead aggregators, or other agents.
