Plain-language information about the Program of All-Inclusive Care for the Elderly.
Last updated: April 25, 2026
The Program of All-Inclusive Care for the Elderly (PACE) is a Medicare and Medicaid program that helps people meet their health care needs in the community instead of going to a nursing home or other care facility. To be eligible, a person must be age 55 or older, live in the service area of a PACE organization, need a nursing-home level of care as certified by the state, and be able to live safely in the community with help from PACE. Costs depend on whether the participant qualifies for Medi-Cal, Medicare, or both; many participants pay nothing out of pocket.
Who is eligible?
To qualify for PACE in California, a person must:
- Be age 55 or older
- Live in the service area of a PACE organization
- Need a nursing-home level of care, as certified by the State of California
- Be able to live safely in the community with help from PACE
Final eligibility is determined by the PACE organization and the State of California, not by this website.
What does it cost?
Cost depends on the participant's coverage:
- If you qualify for both Medicare and Medi-Cal: most participants pay $0 out of pocket.
- If you have Medicare only: you pay a monthly premium that includes long-term care and the Medicare Part D drug benefit.
- If you have Medi-Cal only or neither: PACE may still be available; the PACE organization will explain costs.
We don't quote prices on this site. A PACE organization can give you exact figures based on your coverage.
What's included?
- Primary care, specialists, hospital and emergency care
- Prescription drugs (Part D)
- Dental, vision, hearing
- Physical, occupational, and speech therapy
- Home care, personal care, meals
- Transportation to medical appointments and the PACE day center
- Adult day health services and social activities
- Caregiver support and respite
What about the doctor I have now?
PACE participants generally use the PACE organization's doctors and clinical team. This is a real change for most families, and it's the most common reason a family decides PACE is — or isn't — the right fit.
