    A short call from Leslie

    Leslie Kaz (RHU, CA Lic # 0B32530) calls to confirm a few basics and answer your initial questions. No pressure, no commitment. If now isn't a good time, we'll schedule.

    This week — 1 to 3 days

    Referral application together (15–20 min)

    If PACE looks like a fit, Leslie completes the WelbeHealth referral with you over the phone and submits it to WelbeHealth as your referring agent. You don't fill out a long form alone — we do it together.

    Next 1–2 weeks

    No-cost in-home assessment by WelbeHealth

    A WelbeHealth Enrollment Specialist contacts you to schedule a no-cost visit by their care team (doctor, nurse, social worker). They evaluate medical needs and help complete a Level-of-Care application sent to the California Department of Health Care Services (DHCS).

  4. 4
    About 2–4 weeks total

    DHCS approval & enrollment

    Once DHCS certifies you meet 'Skilled Nursing Home Level of Care' (the federal requirement for PACE), enrollment is processed. You officially become a PACE participant on the first day of the following month. Most participants with Medicare + Medi-Cal pay $0.

While you wait

If you have a Power of Attorney document, recent prescription list, or contact info for your loved one's primary doctor — gather them. The WelbeHealth team will ask, and having them ready can shave days off your timeline. If you don't have these, no problem. We'll work with whatever you have.
